IN MALAYSIA (MISOPROSTOL)

MEDICAL ABORTION

INFORMATION

This page provides information on the process of relying on Misoprostol*.

Initially considering Vacuum Aspiration/Dilation and Evacuation (also known as D&E) as a method, it was then discovered that pills to undergo the process of Medical Abortion is available in Malaysia. The pills, Pfizer's Cytotex (Misoprostol) were obtained via an individual on majalah.com.

Do note that not all Misoprostols are created equal. Research thoroughly on Misoprostol and identify the quality of your seller as best you can.

*The information provides information where the Medical Abortion relies on the usage of Misoprostol alone without the aid of Mifepristone (RU486).
Note: Misorprostol is also widely known under the name Cytotex.

How does it work?

Misoprostol is a medication that works to stop stomach ulcers from happening. It functions by defending the lining of the stomach by reducing the levels of acid that come into contact with the lining.

It has been used in abortion/induced labor as it causes the uterus to contract and the thinning of the cervix. Used as an abortion method, the pregnancy will terminate itself within 6 - 24 hours after the first round of pills are administered.

The pills are best suited for early pregnancies, no more than 12 weeks of pregnancy as the embryo will still be largely comprising of water, fluid and tissues with no bones or muscles having yet developed.

Do NOT take Misoprostol if you have an IUD or an ectopic (extra-uterine) pregnancy.

How do I take the pills?

You can use it in one of three ways: sublingual (under the tongue), buccally (in the space between gum and cheek) or vaginally. It should dissolve within 30 - 60 minutes, ideally within half an hour.

Vaginal routes have higher success rates with faster impression on the body, but sublingual and buccal routes are safer and is more comfortable for the female.

I initially had problem dissolving it (it took over an hour) but subsequent rounds had it dissolve within half an hour as I found better ways. Mine was taken sublingually and buccally.

It is advisable that you begin the ordeal early in the morning as the body will be in shock for the next 12 hours while you experience bodily changes from every intake of the pills. Night time will be ideal for your recovery.

What shall I expect?

The results vary from woman to woman, with the results varying in very large degrees. Some have reported severe cramping, severe diarrhea, chills and nausea.

Please be prepared for such occurrences.
It is advisable that there be a support person to attend to you and be with you during the entire day.

  • FEVER AND CHILLS
In my case, within minutes of every round of pills, my body experienced hot flashes and severe chills. I needed heating pads in my hands, over my womb and behind my back.
Do wear thick clothings and keep yourself warm in preparation of such events.

  • DIARRHEA
Do expect profuse diarrhea when you take Misoprostol. In a study, vomiting occurred in 30% and diarrhea in 50%.

In my case, after every episode of the chills, it was immediately followed by profuse diarrhea.

I have found that eating lightly right before the intake of Misoprostol had helped in reducing the intensity of diarrhea (I ate a few pieces of crackers during the final round.) I took the antecedents on an empty stomach.

  • NAUSEA
Nausea is the most commonly reported side effect, affecting 70% in the sublingual and buccal group and 62% in the vaginal group.

In my case, I experienced nausea only once, after the second round of diarrhea. The combined feeling of nausea and weakness had me almost pass out.
  • PAIN
For some women, the cramps were severe. It is vital that you prepare your body with painkillers (paracetamol, ibuprofen, etc.) around an hour before you take Misoprostol. Taking it together with Misoprostol will not prepare your body in time - chances are the effects of the painkiller would kick in too late and you can be in a lot of pain.

Ibuprofen is the most effective painkiller for cramps.

In my case, there was no pain, only discomfort. It may be because the pregnancy was very early.
I experienced cramps for up to a week as the body slowly sheds tissues from the uterus and cervix.

Remember, some women experienced very bad cramps, so do be prepared in any case. The longer the gestation of your pregnancy, the higher the chances that you will experience pain. Pain is the most common side effect which occurs in approximately 50% of patients and just before they begin to bleed, the pain and discomfort reaches its peak.
  • VAGINAL BLEEDING
The body may start bleeding anytime within 15 minutes to 4 hours after the first round of pills are taken.
Clots may start to be produced as soon as 1 hour.

I bled within the hour, but clots were only produced after 8 hours.

Your body may kick in into its first cycle of menstruation after 3 - 4 weeks after the bleeding has stopped entirely.

If there is no bleeding, or very little bleeding has occurred, the abortion is not successful. In such case, it is essential that you take more dosages of Misoprostol, or head to a clinic to undergo a D&E operation. A continuation of the pregnancy will produce a deformed fetus.

Complications

From various references, you are experiencing complications when these things occur.
In these events, please see a doctor immediately.

  • Have heavy vaginal bleeding and are soaking through two or more large pads an hour, for two hours or more in a row
  • Bleed heavily for more than 12 hours in a row
  • Producing large clots for two hours or more that are larger than an apple
  • Constant vomitting
  • Experiencing severe abdominal pain which cannot be helped by medication, rest, or heating pads

A complete abortion means that the body has expelled all the products of pregnancy (blood, tissue, embryo) and there is no need for surgery later. The only way to make sure that you've had a complete abortion is to have an ultrasound 10 days after using the pills.

Women's experiences have differed greatly, so these pointers are a rough note on what to expect.
